Volunteer & Information Manual 

All school district volunteers are required to submit the following documentation on file at the district office in order to volunteer or chaperone:

1. PA Criminal Record Check (Act 34 – State Police Background Check) – valid for 5 years/60 months from date of issue
2. PA Child Abuse Clearance - valid for 5 years/60 months from date of issue
3. Federal Criminal Record Check (fingerprinting) – only if you have not been a resident of PA for the past 10 years - valid for 5 years/60 months from date of issue
4. Proof of TB (tuberculosis) test screening
5. Volunteer Information Form and signed Disclosure Statement (updated annually) – PDF forms at the bottom of this screen

• Links to obtaining the clearance background checks listed above are provided below. 

   

• The State Police Background check and the Child Abuse Clearance are free for volunteers.  There is a fee for volunteer FBI fingerprinting.

   

• SMSD does offer free TB testing for new employees and volunteers.  Appointments are required and availability may be limited. Please refer to the Volunteer Manual PDF document at the bottom portion of this screen for details on how to schedule TB testing through SMSD.  TB testing can also be done through your PCP or an Urgent Care facility at your expense.

  Please read the attached directions carefully. There is a fee for this clearance and is only necessary if you have not been a Pennsylvania resident for the past 10 years.
